Here's the report ....

I get 36223-50, but I'm not positive - trying to learn the new way of coding these, so any help is appreciated.

PROCEDURE PERFORMED:
Four-vessel cerebral angiogram.

PROCEDURE PERFORMED AS FOLLOWS: The patient brought to the
catheterization lab. The right groin area incised. A 5-French
arterial sheath, through the sheath __________catheter taken to
the level of brachiocephalic trunk, which was cannulated.
Angiogram of the brachiocephalic system with common carotid, internal carotid and also internal and the anterior cerebral middle artery were all obtained. After this, angiogram of the
left common carotid and left external carotid arteries and left
anterior middle cerebral arteries were also obtained.
Nonselective angiogram of the left subclavian system was
obtained. Catheter removed and hemostasis was obtained by
applying pressure.

FINDINGS:
The brachiocephalic trunk is patent, divides into right
subclavian and right common carotid. Right subclavian artery is
patent. Vertebral artery in the right side is dominant and
patent. The right common carotid artery has a stent which is
patent divides into internal/external carotid, both have mild
disease. Right internal, anterior and middle cerebral arteries
are patent. Good venous filling flow of veins is noted.

Left common carotid has a smooth 50% stenosis in two views. Left
internal and external carotid arteries are patent with mild
disease. Left anterior middle cerebral arteries are patent.

Left subclavian artery is a nonselective short which is patent.
Left vertebral is not visualized well.

IMPRESSION:
Noncritical stenosis of left common carotid artery.





CARDIAC CATHETERIZATION

RECOMMENDATIONS:
Proceed with coronary bypass graft surgery.

It doesn't look like th catheter moved from the brachiocephalic. If the physician only watched the run off, then you would only bill the 36221.
