Can anyone tell me the criteria for billing DRG 455, combined posterior/anterior approach? Would it be appropriate to bill a 360 degree fusion under this DRG, (cpt 22612 and 22630)?

DRGs apply only to diagnosis codes and for facility inpatient. Can you clarify what you are needing?
