I have a chart here, in which the surgeon describes plantar warts removed, but the path report says the following: Osteochondral fragments with benign secondary degenerative changes associated with minute fibroconnective tissue and separate fragments of keratotic-type material. 
How in the world do I code that? Any/all suggestions appreciated!
