Wiki Physician visits outside of clinic

New to this forum and really need some additional advice on how to bill, can someone else help me?
I have providers and NP's that go to a Cancer Center when called to visit a patient that is already there for an appointment. It is not a outpatient hospital, just a clinic. Would this be billed with a POS 11 and the usual E&M codes like 99213, 99203, etc?
We also go to a Hospice facility that is independent and a SNF and I am puzzled on what to use for the POS's and the correct E&M codes. Please advise me!
It is alot to ask but I figure there must be someone out there that does these types of visits.

In doing some research, these are my findings:

Cancer Clinic patients POS 49 with E&M 99211-99215

SNF POS 31 E&M 99334-99337

Hospice POS 34 sorry not sure of E & M code

So POS 49 - why I didn't even get that one! That would make sense for sure and then the E&M codes. The SNF I was unsure of those E&M codes so that is a great help and Hospice POS 34 and I will figure out the E&M codes.
