We have our PA's administer an injection when the physician has ordered it and the documentation reflects the PA is the one who administered the injection. Now we are hearing the PA should also have their own documentation for administering an injection.

Can anyone tell me what the correct regulations regarding documentation when this occurs? Is the PA required to have separate note for giving the injection or is it sufficient to be within the physician documentation?

Example:
IN OFFICE PROCEDURE: The left knee injection site was identified and prepped in the usual fashion with alcohol. An ethyl chloride spray was then applied for topical anesthesia. A 22 gauge needle was then advanced to an intra-articular position and aspiration attempt did not reveal blood. Four milliliters of 1% Lidocaine (10mg/cc) and 40 milligrams of Depomedrol were injected without difficulty. The patient tolerated the injection well.

The complete note is signed by the Physician.
