I've seen this a few times recently, and every time, there was also a remark code N572, which means that the Functional Status Reporting codes were not billed on the same claim. The only other modifier I can think of would be GP, but that would be the equivalent of GO, which you are already using.

It is definitely something with your G codes, either the claim is out of sequence for the every 10th visit or another claim was that's causing this one to kick out. You'd have to call them to figure out exactly.
