Wiki Physical Therapy LCDs

Looking for a reference list that has CPT codes for physical therapy modalities and then lists each diagnosis under that modality.

I searched the LCDs today and found this message: The list of KCD-9 codes that directly relate to each CPT code has been removed from the
LCD. The medical record must document both services performed ant the reason for the
services.

I'm hoping someone has a list that is not too outdated.

Hi Jenny, Which contractor jurisdiction are you under(i.e which state and is it part a or part b)? Some contractors give lots and lots of detail and some don't anymore (super frustating). Let me know which state you are in and I may be able to help. I work on LCD/NCD data all day long.
