I have a question. My employer want me to audit PT & OT notes. there is special tool to audit these notes? I can't seem to find any info. Please Help!
 
No there is no audit tool. Make sure time in and time out are documented. Also check and see if functional goals and progress or lack of progress are mentioned. All exercises and therapies should be clearly documented as well as checked off. Time for manual therapies should be clearly documented. If less than 8 minutes, you can't charge for it. It will have to be bundled into other therapies. Also make sure there is a signed order or progress note for more therapy with time or visit limit on file. All notes should be signed by the therapist.
