My doctor saw a patient today who had her annual exam, but he also took out her pessary, cleaned, irrigated, and then reinserted the pessary. I know on a normal basis for just a pessary cleaning we would bill the E/M code, but is there anything I can bill with an annual to get the extra charge for the time and work of the cleaning?

Hi Ashley

E/M 9920_ for new pt's or 9921_ for established pt's need a modifier of 25 w the proper dx for the pessary.

For the annual depending on the insurance you may use the preventive codes the 9938_ for new pt's or the 9939_ for the established pt's along with the icd v72.31, v76.2 or v70.0.

For Medicare pt's you will need to use the G0101,GA and the Q0091,GA (GA denote that pt sign an ABN) with the icd v72.31, v76.2. This aren't the only dx that you can use for the medicare pt's. Check the Medicare site for more details.
