Procedure: Exploration of right wrist with attempted fistula placement
Diagnosis: ESRD
A 0.5% Marcaine was infiltrated halfway between the cephalic vein and the radial artery. Carried through subcutaneous tissues. I moved laterally. Identified the cephalic vein, which was obliterated, sclerosed and thrombosed. I initially felt that it might be usable for dialysis, but without lumen, it was impossible. Irrigated with normal saline, controlled bleeding prior to closure. Closed in 2 layers....etc.
Unsure of what code to use. Since surgeon did not complete procedure would I bill with mod 52 for reduced service? mod 53? for discontinued service? Unable to find anything for exploration of wrist. Would appreciate any assistance. Thank you!!!
