Wiki Periapical abscess K04.7 insurance denials

Coding for primary care not dental office. Would anyone know how to get K04.7 paid? Getting many denials on this code. Does the pain code of K08.89 need to be primary? (if pain is documented for reason of visit of course) Possibly does it need Z13.84 encounter for screening for dental disorders added? any advice would be apppreciated. Thank you!

If pt has jaw pain I d add it R68 dx if listed in documented. Also add K12.2. You can bill Excludes 2 rules dx codes , but Z dx last

Hi, Jaw pain is not documented so I wont include that but dental pain is documented K08.89 as chief complaint. Are you saying to add K12.2 onto all of my periapical abscess K04.7 codes? followed by the Z13.84 code. So My claim would look like this. Or would I change the K04.7 to K12.2? Thank you for helping!

If the dental gum abscess is documented I d add it first the K12.2 or K04.7 per notations then follow Z13.84 last. It always help if provider gives you better dx than unspecified dx.
