NCCI guidelines state '
shall not be reported separately with an open or arthroscopic joint procedure when performed on the same joint. However, if an arthrocentesis procedure is performed on one joint and an open or arthroscopic procedure is performed on a different joint, the arthrocentesis procedure may be reported separately.'

What about Perc procedures such as CPT 27306?

Perc Patella Tenotomy with US guidance. Dr. also wants to bill 20611 for Hydrodissection of Hoffa Fat Pad from posterior patellar tendon. 5 ml lidocaine injected for local anesthesia.20 ml IV fluid used to hydrodissect the posterior patellar tendon off the Hoffa fat pad."
