I have a pelvic ultrasound that was dictated but was ordered as a US OB limited 1 or more gestations.

There were no fetal movements. 32 week 1 day fetus by estimated date of delivery.

IMPRESSION: Findings consistent with fetal demise.

Would this code be 76815 or 76857?

Having trouble understanding OB ultrasounds.

Hi deb. You have to code with 76815 whereas you cannot code with 76857 because its a nonobstetric code. Any views from your end would be greatly appreciated. Thanks
 
This is an obstetric ultrasound. The patient was 32 weeks pregnant, the fetus has expired but that does not make it non obstetric.
 
Ob vs Non Ob

CPT states, through its companion publication, the CPT Assistant,5 that the use of OB ultrasound codes are appropriate anytime the patient has been established to be pregnant, by any method or means, and the indications for the ultrasound study might be or could be pregnancy-related, regardless of whether the outcome might not confirm a pregnancy, or might produce a diagnosis that is not pregnancy-related. For the patient who has not been diagnosed as pregnant, and the indications for the ultrasound study are not pregnancy-related, the use of non-OB ultrasound codes is appropriate, regardless of whether the outcome of the study shows a pregnancy or complication related to pregnancy.
