My doctor is doing a peritoneal dialysis catheter salvage (cuff shaving) procedure in the office. I'm not sure how to code this. Initially I told him it would be included in the E/M code, however, he feels it should be separately coded as it takes about an hour to do. I am leaning towards 49999 but not sure what code to compare it with, there is 49325 Lap revision but that is done in the OR. Has anyone come across this yet?

Procedure: peritoneal dialysis catheter salvage procedure
Sterile prep with chloraprep. Draped. Local anesthesia with 10 cc of 1% lidocaine with epinephrine. The skin was opened over the peritoneal dialysis catheter. Purulence was identified and cultures were obtained. The subcutaneous cuff was mobilized and then cuff shaving was performed using a 15 blade. Hemostasis was obtained with cautery. The wound was partially closed with 3-0 Biosyn Dressed with biopatch and gauze.
