Patient had a forearm fx 4/13/2013 with a closed reduction. Patient has returned on 06/26/2013 with a refracture same side and site he is still in the global. Would you bill it with a an E/M modified and the casting supplies or for the casting application and E/M modified

Refracture sounds like a new injury so I would report the E/M. Just be ready to appeal.
 
Code for the e/m with a modifier 76 or 77 whichever is appropriate as well as the same CPT code that was used before. This tells insurance that the procedure being performed is an exact replica of a procedure that has occured which instituted the global in the first place. In addition, do not forget to change the patient's date of injury to reflect the necessity of re-treatment.
