Hello,

My Medicaid bills are being denied with the remark code MA04. According to Medicaid they request I submit a claim using Payer code 16. I contacted my billing software Amazing charts and they do not know how to proceed in putting in this code in my claims. Can anyone help me with this? Seems Amazing chart is not familiar with this.
