A physicians is asking if they can charge an insurance company or a patient for walking out of the office after being put in a room and a note started, if they have been waiting to long? Any idea's?

You can only code and bill for a service that is performed - if no services have yet been provided, there is nothing to submit. You could consider something like a no-show fee for this type of situation, but I would strongly recommend against it as poor customer service. If the patient came for their appointment and didn't get the services in a timely manner, the patients would surely object to any kind of charge and only thing it would likely accomplish would be to aggravate the patients more, generate complaints that your staff would have to deal with, and potentially harm the reputation of your practice.
