Good Morning,
with regards to billing a sick visit with a preventive...
It's better if the doc does them on seperate visits, but in the event that he does both on the same day I would advise the doc to to an Addendum to the preventive note and clearly document what the problem was that he addressed while the patient was in the office for their preventive visit.
Modifier 25, would get appended to the OV E/M code, not the preventive.
