If the pathologist is doing 88321 or 88325, while billing these services, the DOS should be the date on which sample was collected or the date on which the sample was received?

that was a great site you recommended.

I would like to ask the following:
1. 88329 - 88333 Are these only to be used if consult provided "during surgery"?
2. Is this literal and the pathologist is required to be in the OR?
3. if so, what consult code can be submitted if specimen was physically taken "during surgery" to the pathologist; who was not in OR?
4. Can consult code 88331 be submitted 2x for two specimens received on the same day and same patient?
