It looks like a documentation problem. But the question, did the provider just removed the skin tag that is why the is no record of sending it to the path lab? May be your code is not biosy but just excision. If their is no record of biopsy result you cannot bill for biopsy. In biopsy provider have to obtain tissue sample and sent to lab for separately reportable path exam. Biopsy, provider just take a small tissue sample compared to excision, provider take all the skin tag with or without sending it to the path lab.
