Wiki path charges for Medicare

Hi everyone,
Is anyone else having a problem with medicare denying path charges saying that another charge has been submitted for the DOS you are billing for? Usually this is a hospital and when patient is contacted they were not in the hospital on that exact date? I think what is going on is the span that is used by hospitals for weekly visits, chemo, PT, etc.
I am billing 88305 with TC. We see pt one day, do a bx and bill next day for path.
Any help would be appreciated!!!!

Are you the office doing the biopsy or the pathologist reading it? The pathologist will bill the 88305 not the MD performing the biopsy.
