I am having difficulty finding documentation to support that it must be the physician that perfoms all of the elements required for an initial visit for a new patient to establish a plan of care. Can the PA of NP perform any part of the visit and the have the physician establish the plan of care and still be billed under just the physician MCR number. Any help or direction on this is greatly appreciated. Thank you
 
In the Medicare Claim Processing Manual, chapter 12, Section 30.6.1 B, Split/Shared E/M Services, Office/clinic Settings you'll find your answer. This paragraph states that if both the MD and NPP see a patient the encounter can be billed under the provider if "incident to" criteria is met and the patient is an established patient.
