Hello..
I am having a problem with Noridian paying for Injections. I am billing for the ASC.... Example 64483 for L3 to L4, and 64484 L4 to L5, and 64484 for L5 to S1... They are paying the first two but not the third or just the first and not the second two... I have never had a problem until recently did something change that I am unaware of and can't find an answer to??

As of January 1st, 2014 only the primary code is payable to the ASC. The add on codes have been bundled and are no longer payable. Look at the ASC approved procedures list and you will see that the add-on codes are no longer listed for separate payment.
