I was taught to not code signs and symptoms, but would like another opinion... if a patient has chronic pain in his knee and had a knee replacement do we code

G89.29 other chronic pain
Z96.652 Presence of left artificial knee

OR

G89.29 other chronic pain
M25.562 Pain in left knee

OR all three codes??

What is the reason for the visit? Follow up of the joint replacement? Or pain control?
