I have been having A LOT of denials from various insurances for codes 64400-64455 and also for the Ultrasound guidance 76942. I had been able to send an appeal with a copy of the Chapter 2 Anesthesia Services NCCI Edits and medical records and have been able to get most of them paid. Recently not even my appeal has been able to grant payment. I don't know if anyone has been having issues, per the NCCI edits there are no changes on how to bill this. I am also not finding any other forums anywhere else. We even have denials from Humana to add LT/RT modifier for anatomical purposes which has not been an issue before.
If anyone has any suggestions please let me know.
