If the provider, when documenting the p.e., states:
MUSCULOSKELETAL: Moving all extremities without obvious restriction in range of motion. No deformities. No extremity tenderness. No joints effusions.
PERIPHERAL VASCULAR: Calves are non-tender. Extremities are warm and well-perfused without clubbing, cyanosis, or edema. How many bullets would that be under the 1997 guidelines?
Under these two headings how would you count these and how many points would this be just for these two headings?
