Wiki OV with procedure and injection

I work with neurosurgeons. They do alot of procedures such as 62311, 20552, 64494 with 77002 and J3301 (kenalog usually 10 mg x 4) with an ov. Would I put a 25 modifier on the ov and 59 on the procedure. I'm not sure how to code these. Can I bill all three to medicare?

It really depends on the provider's documentation. Can you post an example?
 
You can bill an ov with procedures using just he 25 modifier, as long as the documentation supports this, as far as need a 59 on the procedures depends on whether 2 or more procedures are bundled which you will need to check on. If you have an ov and only 1 procedure then there is no need for the 59 modifier.
