Wiki OV and diagnostic ultrasound

I have been trying to get 99213/99203 and 76881.59 paid but always comes back bundled no matter if I use modifier, separate dx etc. Anyone know how/if I can get these 2 paid when done on same day?

We bill modifier 25 with the office visit and left or right on the 76881 and do not have any problems getting reimbursed.
