Wiki outpatient transferred to inpatient

Pt was admitted to the hospital from an outpatient encounter after receiving a neupogen injection with adverse reactions. Same MD saw the pt in the office as well as admissions. Can I bill the injection performed in the office with the initial hospital admit code using a 25modifier and the supporting office notes.
