Wiki Oswestry Disability Index & Visual Analog Scale/Score

Hello!

My Orthopedic Spine Doctor asked me if I can bill an Oswestry Disability Index or Visual Analog scale along with an E/M code. I believe these are just tests performed during initial exam, and I don't believe they can be billed separately. Does anyone have any ideas?
