Need help in coding procedure Right first dorsal wrist compartment lengthening and tenosynovectomy OP Note is as follows:
Postoperative Diagnosis: Bilateral thumb stenosing flexor tenosynovitis, right de Quervain's tenosynovitis.
Procedure Performed:
1. Right first dorsal wrist compartment lengthening and tenosynovectomy
2. Right thumb A1 pulley release
3. Left thumb A1 pulley release
A transverse incision was made on the R thumb MP flexion crease. Dissection was carried through the subcutaneous tissue. The digital arteries and nerves were protected. The A1 pulley was released longitudinally. Mechanical catching resolved with a passive motion. The first dorsal compartment was opened in a Z-lengthening manner after a zig-zag incision. There was marked thickening of the tenosynovium, a tenosynovectomy was performed. The two flaps of the retinaculum were transferred and sutured to each other, nicley lengthening the first dorsal wrist compartment.
Now the left hand was addressed through a transverse incision in the MP flexion crease of the thumb. Dissection was carried through the subcutaneous tissue. The A1 pulley was identified and released longitudinally. There is no mechincal catching through range of motion.
