Our orthopedic department is in the process of revising the forms used to document patient E&M services and I would appreciate it greatly if anyone can share a copy of their orthopedic form. Our current forms are very outdated (no pre-printed info to assist the providers with documentation) and I would like to show them a few examples of templates so they can determine how they want to design the revised form. I need these as soon as possible because decisions have to be made soon.
