I don't normally code ortho. I have someone asking me to check the coding on 2 reports and I have a nagging question I can't find an answer to. Patient came in and had a bunionectomy and osteotomy with screw insertion. A few months later it had delayed union. The doctor is now doing a "hardware removal of the osteotomy site and ORIF of the osteotomy site using a T-plate. He's trying to bill a 28485 on the second procedure and I'm wondering if the ORIF codes were meant only for fracture care? Patient never had a fracture, but there is a delayed union. Should I be looking at something more along the lines of 28322?
