Can I use V58.11 for a patient on oral chemotherapy when he comes in for a follow up visit or would V58.69 be more appropriate?

As long as the encounter is for monitoring of the chemotherapy I think V58.11 is more specific.

The encounter is for follow up to treatment. MD may increase or decrease dosage dependent upon efficacy or side effects of medication.

Because you are talking specifically about chemo drugs, I believe it is recommended to use the V58.11 as it is more specific. The other codes would also work, but are less specific to the type of therapy.
