I have a dictated note where a medial and lateral menisectomy was done ,the note states patella was also noted to have grade 3 to 4 patellar chondromalacias. A small microfracture technique was then carried out in this area to allow for growth of fibrous cartilage. My question is this enough documentation to support a microfracture?

Yes, I would bill for microfracture on that.

I like to see documentation that supports debridement down to bleeding bone but this may be sufficient...depending on the carrier. Makes it easier for appeals, if needed.
