A refraction is rarely a covered service at any time. Lasik surgery is a not a medically necessary surgery and therefore is usually paid out of pocket. I know there are some insurance companies that offer a rider on their policies to cover this procedure but very few. When you pay out of pocket for a procedure usually all post op visits and testing has been considered and paid for prior to the surgery and a refraction would be considered part of the post op work up.
