Wiki Open and arthroscopic rotator cuff repairs?

Does anyone know if there is some official rule regarding when a surgeon performs both a mini-open cuff repair and an arthroscopic repair? Has anyone billed for both and been reimbursed?

The surgeon performed an arthroscopic repair of the subscapularis and then a mini-open repair of the supraspinatus.

Per NCCI edits they are bundled you cannot bill them together same shoulder.

I agree once you convert a scope to an open procedure for the same repair, etc...it is included and use v64.43 as the 2ndry dx.
