Can someone clarify as to when I would use the regular CA codes (i.e. 195.0) and History-of codes (i.e. V10.3)? Thanks.
History of, is used when there is no evidence of disease and no treatment being given, otherwise you would use the regular Ca code.
 
If the patient is still undergoing treatment you use the Ca codes, if the patient is no longer receiving treatment but there is still cancer present you use the CA codes. If there is no further treatment abd there is no evidence of disease, you use the HX of codes. Remember drugs like Tamoxifen and Lupron treat cancer cells for many years after all other treatment has ended, and the provider declares no evidence of disease, however it is still coded as active cancer since that is what the drug is treating.
