How do I code for a old fracture found on an xray? Is this a hx code?

How do I code for a old fracture found on an xray? Is this a hx code?

It would depend on the nature of the fracture, but in general I'd code it as an initial encounter. It's no different from coding a fresh fracture - find the correct bone, then the correct side, then the correct type of fracture, then the encounter type.
For example: femur, right, closed, initial, would lead you to (a quick search) https://www.icd10data.com/search?s=fracture of right femur, initial closed S72.8X1A.
This is all assuming that the patient has had no treatment for the fracture. If they HAVE been treated previously (or examined previously), then it's either a subsequent encounter or sequela encounter. If a provider attempted to treat the fracture previously (set, ORIF, etc) and it still shows on imaging as a fracture, then it's delayed healing or nonunion.
