Wiki Office visit with fluoro guided injection in Radiology

We are in a quandary & need guidance! Ortho provider examines a pt in the office, takes the patient to radiology to perform fluorosopic guided injection of contrast. Pt has arthrogram of shoulder. Can the provider bill an E/M level of service as well as 23350, 77002, & 73222? My concern is the provider going to the radiology dept.

Was the procedure planned? Ours are always scheduled in advance so no E/M is necessary.

Any situation you would have to put the 25 on, so can you say it was separate & distinct?? Has he evaluated this problem in the past? If he has there is no reason for the E/M he already knows what is going on.
