Wiki OD AND MD same visit-how to bill?

I am looking for advice and/or resources to advise a practice on how to bill for a visit, where an OD saw a patient and MD supervised. BOTH sign the note, and we get sporadic rejections stating MD cant bill b/c OD did. I am new to eye coding and more familiar with PA/NP bulling incident-to. help and thanks

They have to bill the visit for one doctor or another, not both. If the OD is licensed, why is the MD "supervising" them? That doesn't really make any sense to me and just adds confusion as to who provided the care to the patient.
