I am lost in trying to code an observation visit on a patient that had a home delivery. She delivered both the baby and placenta at home, and was brought in by ambulance to be checked. this is what the note reads:
"the patient was delivered at home and brought in by EMS. The placenta delivered spontaneously at home, was examined, and found to be intact. The perineum was intact, with superficial nonbleeding perineal laceration. There were no known complications and the patient did well."
The patient was discharged same day.
