I code OBGYN and when the delivery note states Michal cord easily reduced. I code by hard cord without compression. I am now being told from auditors that this was not complicating the delivery so I should not code it. Does anyone have any kind of supporting documentation for this?

Any condition occurring delivery or pregnancy is considered a complication of delivery or pregnancy. The nuchal cord is definitely a complication whether it was compressing the baby's neck or not. The only time a condition is not a complication is when it's documented as incidental. My reference is the first paragraph of the Chapter 15 in the ICD-10 manual.

Thank you. We have a new auditor and I am getting some interesting instructions. They are also telling me I can’t code elderly multigravida unless the doctor notes AMA. I have been coding OBGYN for 12 years and have never been told this.

If the patient is over 35 years old and prego and had other births she is a Elderly,” in the language of obstetrics, refers to pregnant people who are 35 or over. (Multigravida means I had been pregnant more than once. The notations can tell you this data...it can be implied in documentation. Do the auditors know .....a high-risk pregnancy is a pregnancy that involves increased health risks for the pregnant person, fetus or both. Certain health conditions/chronic conditions dx 010 block and your age (being over 35 or under 17 when pregnant) can make a pregnancy high risk. These pregnancies require close monitoring to reduce the chance of complications. However the provider should note down if patient is high risk . If patient under 16 yrs old pregnancy then dx 090.6 block.
