Pt has a pelvic under anesthesia due to not being able to tolerate procedure. She also has a rectal exam (just rectal no scope or anything like that). What code would I use for the rectal?

It would be included in the pelvic or e/m done that day. Unless you use a scope I do not believe it is separately billable.
