Pt admitted for "anemia and HIT(heparin induced thrombocytopenia). Anemia multi-factorial (CAD, CHF, PAF s/p PPM). Seen in consult by hematologist.

My question is under MDM-number of dx's or treatment options; New pt no additional workup planned (max=3 points). If there is more than 1 dx and you're limited to 1 for no additional workup where could/would you count the other dx's?

Risk is high due to the other co-morbidities. Data review is low; labs and scans reviewed. I feel the physician could/should get a high/MDM but can't due to max for no additional workup planned = 3.
 
Any documentation of your doc discussing the case with the hematologist? Ours always document if they spoke with the consulting specialist, and for that first discussion, I give the two points for discussing the case with another MD. That could help get the data points up to the high MDM level.

Should have been more clear on my question. My doc is the consulting physician.
