I have a provider who did an arthrodesis from T11-pelvis. He noted in his op note "T11-T12, T12-L1, L1-L2, L2-L3, L3-L4, L4-L5, L5-S1, and S1 pelvis". When I asked him how far he went down into the pelvis (sacrum) he told me that he went to the pelvic crest (iliac crest) and that it adds on level and he inserted pelvic screws in the top of S1. He noted 22614 x 7 but I am getting 2216 x 6 as you can not use the iliac crest as a segment as it falls between L3-L4 usually. Can anyone help with this? He is also a provider who needs facts behind it so I cant just go to him and say "so and so said this" I need facts.
