Wiki non union of ulna shortening?

pt had an ulna osteotomy 3 wks prior, due to a nonunion removal of hardware and replace had to be done. Would 25400, repair of nonunion or maulunion be the best cpt code? Or should I just use the 20680 w/ 22 modifier?

if you're removing old hardware + replacing that hardware (screws, pins, etc.) i would stick with 20680-22.
