We are finding that insurance companies are now bundling our non-ob ultrasounds (76830) with any E/M service billed. They are instructed us that this began in Feb 2011 and we should append modifier 25 to the E/M code. Otherwise, the insurance pays the u/s and denies the E/M service.
Anyone else having this problem? Do we know where/why this originated?

We're having the same issue with UHC. I believe it's one of the changes that came about with most recent NCCI edit. If I remember correctly an E/M visit is now considered a intregral component of the U/S procedure, so unless the visit if for something unrelated-they're going to bundle it.
