If a patient is 17 years of age and has BCBS, and speaks to an MD on telephone and meets all other criteria for a telephone visit, is this billable as a 99441-994493 range?

Would you have seen the patient in the office if they walked in for an appointment without their parent? Whatever your answer is for that, do that for this.
 
In this case, though, it's water under the bridge - too late to not see the patient. But really it is a question that should be directed to the practice manager, not the coding forum, if there's a concern that physician has potentially treated a patient without the legal authorization to do so.
