My physician often states "Patient seen and evaluated with no new complaints" (these are rehab patients receiving PT and OT). I would like your opinion on whether or not this statement can be used as a ROS? Thank you.
I would not use that as ROS, no. There may actually be a bigger issue here. Are these visits happening on the same day as the treatment? If they are coming in for PT/OT and this is just a quick pre-treatment exam, you probably shouldn't be billing the OV's at all.
These are inpatients at the rehab unit of the hospital, and we are charging subsequent hospital visits, my physician always does a small HPI and does a good exam, but is lacking in the ROS department.
